CANCELLATION POLICY
A cancellation fee of $30 will be billed to you if you do not provide 24 hours notice prior to cancellation of your appointment.
If you miss an appointment, you will be required to pay 100% of the service total.
Medical Records
Your personal and health information is preserved and protected under federal guidelines. No information is released without your permission or as permitted by law.
Payment
Unless other arrangements are made, payment is expected at time of service. Payment can be made by cash, check or credit card. Fees for services are detailed on the Services & Rates page.
Client Forms
Please print off the following forms and bring them to your appointment.
Please print out and complete the following Client Intake Forms (2 page document) and bring them to your session. If you are unable to print them out, forms will be provided upon arrival.
The following form will help you clarify your goals for your overall health and vitality, including our time together. This form is not required, but suggested if you would like to track your progress.
Youth Parental Permission Form
Form required for youth under 18 to receive treatment.
